Extended Warranty Coverage

Forest Medical, LLC warrants the Trillium™ Holter Monitor to perform according to manufacturer's specifications for a period of one (1) year from the date of receipt of payment of the Extended Warranty. Forest Medical, LLC will either repair or replace any components found to be defective or at variance from manufacturer's specifications within this time. Each Trillium Recorder may be returned to Forest Medical each year for inspection, cleaning and general maintenance at the customer’s discretion during that time. It shall be the purchaser's responsibility to return the instrument directly to Forest Medical, LLC, post-paid. This warranty does not include cables, batteries and other accessories. This warranty does not include breakage or failure due to tampering, misuse, neglect, accidents, modification, or shipping. This warranty is void if the instrument is not used in accordance with the manufacturer's recommendations. This warranty is void if unauthorized or unqualified persons attempt to make repairs. The warranty entitles the owner of the equipment to replacement or repair of the part(s) or unit at Forest Medical, LLC's discretion. Purchase date determines warranty requirements.

THE WARRANTY PROVIDED ABOVE AND THE LIABILITIES AND OBLIGATIONS OF Forest Medical, LLC PROVIDED FOR THEREIN ARE EXCLUSIVE AND IN LIEU OF ALL OTHER REMEDIES, WARRANTIES, GUARANTEES, OR LIABILITIES, EXPRESS OR IMPLIED, ARISING BY LAW OR OTHERWISE (INCLUDING WITHOUT LIMITATION ANY WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE AND CONSEQUENTIAL DAMAGES) WHETHER OR NOT OCCASIONED BY Forest Medical LLC’s NEGLIGENCE.

Please return the instrument registration card for warranty validation.

In case of a defect, or other warranty service, please call Forest Medical, LLC for a Return Material Authorization (RMA) Number and return the unit post-paid directly to the manufacturer. No returns will be accepted without an RMA number issued by Forest Medical, LLC. Mark the RMA number on the outside of the package and return to:
